6 Veteran/employees believe that a provider entered their record inappropriately. We are reviewing sensitive access logs and patient records to see if there was a compelling business need to be in the record. Update: 05/24/12:The provider did not go through appropriate procedures to access the information. The provider is a HIPAA covered entity who did have a buisness need to know the information, however did not follow proper procedures to access the information. This is a policy violation and not a security breach.07/30/12:Upon further investigation, a HIPAA notification is required in this case since they inappropriatley accessed the 6 employee's medical records in error.

Provided Training/Education to Employee health team on proper access to Veteran Health records.
