Issue:

A VA employee was arrested on 9/5/2011 by county (non-VA) law enforcement. Found in his car and in his home was an "inpatient bed roster" dated August 23, 2011 containing room number, full name, full SSN, age, admission date and some handwritten notes on the paperwork containing medical information on some Veterans. There were 30 Veterans on this roster. Additionally, non-VA law enforcement recovered prescription bottles containing patient information (they are being held for evidence). At this time, it is unknown whether all patient information has been recovered. VA police are trying to obtain a search warrant. Update: 09/06/11:Forty-six (46) Veterans will be sent letters offering credit protection services due to full name and full SSN being exposed.

Outcome:

Computer/network access has been disabled and the employee has been referred for further action.
