Issue:

On 03/25/15, an Environmental Management employee was working on a loading dock disposing of cardboard boxes when he came across three envelopes. Upon checking the envelopes, he found that two of them were empty and the third contained an Appointment List of a Veteran. He confiscated the documents and provided them to his Supervisor, who alerted the Privacy Officer (PO) of this. At this time, it is unclear if the Veteran may have disposed of the envelope and document or if a VA employee was at fault. The PO will be contacting the Veteran to ask if he disposed of this document. If not, then this will be escalated to an incident at that time.

Outcome:

3/30/15: PO update -- 3/30/ - PO called and spoke to the Veteran whose information was on the document found. He stated that he receives all of his care at the Nashville Campus and was not at the Murfreesboro Campus last week when the document was found. PO will now investigate to see if can be determined who might have discarded this document. 03/30/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
