Issue:

Patient A says that on Wednesday 01/18/12, he was in Audiology to schedule an appointment. The patient says he was talking to Employee A when her personal phone rang. The patient says the employee told him she had family issues and had to answer it. the patient said she answered it and told whoever it was to hold on and then laid it down with the ear phone up and asked the patient four his last four digits of the SSN and other personal information. The patient stated "I dont know who was listening on that phone and thats not right." Update: 02/02/12:The investigation has been completed. It was confirmed that the employee did in fact compromise the patients' last name and the last four numbers of his SSN.

Outcome:

A recommendation has been made to the supervisor for appropriate administrative actions to be taken against the employee as well as refresher training.
