Issue:
Veteran A called and notified the VA Maury County Outpatient Clinic, that when he received his Patient Notification of Test Results letter, he also received a Patient Notification of Test Results letter for Veteran B. The Privacy Officer (PO) was able to confirm that, at the minimum, the letter belonging to Veteran B contained his full name, home address and test results. The PO will be contacting Veteran A to arrange return of the Veteran's B letter.
Outcome:
01/22/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
