Issue:
A Patient called a community out based clinic to report that they had received another Veterans appointment letter in with theirs. The appointment letter contained the Veteran's name last four numbers of the social security number and address.
Outcome:
02/27/14: The Incident Resolution Team has determined that a Veteran will be sent a notification letter.
