Issue:
The Imaging registration clerk printed a wrist band for Veteran A and placed it on his wrist. During the verification process, it was discovered the information on the wrist band was not Veteran A's but Veteran B. The wrist band contains Veteran B's full SSN and full name. Update: 05/06/11:Veteran B will be sent a letter offering credit protection services.05/09/11:The PO reported that it is unknown if Veteran A's wrist band was printed and put on another Veteran. The Imaging Registration Supervisor is unable to determine if this had occurred. This PO is requesting credit monitoring services for Veteran A also. Therefore Veteran A will receive a letter offering credit protection services.
Outcome:
A Privacy Violation Memo has been sent to the Supervisor and Human Resources.
