Issue:
Veteran A contacted the pharmacy today to report that Veteran B had mailed him a letter along with prescription paperwork that belonged to Veteran A. He contacted the pharmacy to be sure that all of his medications were correct in our system and to advise us that this error had occurred. The pharmacist apologized on behalf of the medical center and assured him this error would be reported and taken very seriously. Veteran A then expressed that he knew that with that kind of information anyone could access personal information to find SSN and other personal information. The information at risk included Veteran A's name, address and medication information. Update: 09/04/12:Veteran A will receive a HIPAA letter of notification.

Employees provided continued education for privacy awareness.
