Issue:

Two envelopes were prepared with medical records for Veteran A and Veteran B to take to their respective appointments. Veteran A received Veteran B's envelop and Veteran B received Veteran A's envelope. Veteran A took his envelope to a VA different VA facility where the records were destroyed before the Veteran was made aware. Veteran B is in the process of returning the records.

Outcome:

09/29/14: The Incident Resolution Service Team has determined that Veterans A and B will be sent a letter offering credit protection services.
