Issue:
On 03/11/13 at 1:30 PM, the Privacy Officer (PO) received a phone call from Veteran A stating that while visiting the Beckley VAMC on 03/05/13 in the Orange Clinic, she requested a copy of her upcoming appointments at the scheduling desk. Sometime after she got home she was looking at the list of upcoming appointments provided to her by a scheduling clerk, and she realized that she had Veteran B's list of appointments instead of her own list of appointments. Update: 03/11/13:Due to full SSN and medical information being exposed, Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of # Mis-Handling incidents this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
On 3/11/13 supervisor over the scheduling clerks emailed the clerks (and copied the PO) reminding them that before giving out any paper that contains any personal information to a patient to take extra time and verify by at least two identifiers (full name, date of birth, or full SSN) that they are giving the information to the correct patient.
