Issue:

A medical doctor from United Health Group mailed Veteran A's Medical Records consisting of 200 pages through the United States Postal Service (USPS) via Priority Mail to the Cincinnati Medical Center for scanning into the Veteran Record. According to the USPS, the package was found loose in the Mail Section at the Cincinnati Network Distribution Center. During the course of processing mail items occasionally become separated from the envelope or package and sustain damage. Cincinnati VA Medical Center received the package sealed from the United States Postal Service. When the Release of Information (ROI) clerk opened the package, the USPS notification note was found indicating that the item was loose in the USPS Mail Section. Update: 08/19/11:There was a USPS delivery issue, however because the full SSN was exposed. the Veteran will receive a letter offering credit protection services.

Outcome:

No corrective actions taken, since this occurred at the United States Postal Service. Credit monitoring letter has been sent to Veteran.
