Issue:
Veteran A called the Pharmacy to state that he had not received his medications. The Pharmacy tracked through UPS and determined the medications had been shipped to the Veteran's old address. The person at old address was contacted but denied receiving the medication however the medication at the old address was signed for. The medication was never recovered. An internal investigation is ongoing. The information at risk included Veteran A's name and medication information. Update: 03/12/12:Veteran A will receive a letter of notification.
Outcome:
Staff were education on accuracy of addresses.
