Based on record review, the facility failed to protect the private health information (PHI) of Patient A. This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 5/28/13, was reviewed on 5/28/13. The report indicated Admitting Clerk 1 gave another Patient a condition of admission and Patient Rights papers to sign. The forms were labeled with Patient A's information. The labels contained the following information: Patient A's name, date of birth, medical record number, and account number. The facility investigated the breach, and provided training and counseling to Admitting Clerk 1.
