Issue:

On 04/28/11, Veteran A (Social Security Administration employee) received the results of her lab work by mail. There were two additional lab results, which were intended for Veterans B and C, included in the same envelope. The letters contained Veteran B's and C's name, full SSN and lab results. Veteran A contacted the Greenville Outpatient Clinic to report the incident. The Chief of Release of Information is being notified. A formal letter requesting that Veteran A return Veteran B and Veteran C lab results to the Dorn VAMC was mailed on 04/29/11. The information disclosed: Full names, last four digits of the social security number, address, lab results, and the name of the providers. Update: 04/29/11:Veterans B and C will receive a letter offering credit protection services.

Outcome:

Staff is being re-educated on the need to safeguard patient information.
