Issue:

A private company received a fax of a Veteran's clinic schedule. The fax was shredded and the facility notified. Update: 05/09/11:The elements that were reported were the name and SSN. The faxed sheet, which was shredded, looked like a clinic schedule with one person's name and SSN, per the POC from the company reporting the incident to this facility. The title of the faxed sheet had 'Tamp VA/Bond' or 'Veterans Health Clinic/Bond'. Health Administrative Services researched all Tampa VHA clinic names and there is no clinic by that name nor a provider by that name associated with any of the Tampa clinics. They also ran the same information by the fee basis section and it too was negative. There are 18 Veterans in the system with the indicated name. None of them have appointments scheduled within the next 2 weeks. There is a chance this may be a Orlando VHA clinic as the company is located in Winter Haven. The Privacy Officer (PO) called the Orlando VHA PO and he is researching the same information.)5/13/11:The investigation revealed that it was from a VA clinic. The Veteran will receive a letter offering credit protection services.

Outcome:

Supervisory chain of command notified of incident. Direct supervisor reviewed situation with clinic staff and reviewed correct procedures for faxing of PHI and III to off site locations. PO will conduct follow up training at CBOC site with all admin and clinical staff.
