Issue:

Based on interview and administrative document review, the hospital failed to keep personal health information confidential when Patient 10's Medication Reconciliation Record was given to another patient. The patient who received the information was not involved in the patient's care and not authorized to receive the information. This failure resulted in a breach of personal and medical information and possible unauthorized use of the information.Findings:In an interview on 7/30/13 at 11 a.m., the Compliance/Privacy Officer stated that, on 5/15/13, medical information of Patient 10 was given to another patient, who was being discharged. He explained that the Medication Reconciliation Record was given to the patient in error and confirmed the information which was sent.Review of administrative documents revealed that the information revealed included patient's name, weight, date of birth, Dr. name, date of service, medical record number and a medication list which included the name of medications that the patient was receiving. The facility explained that a printer jammed and when cleared, this document was produced and inadvertently placed in the other patient's folder. The employee failed to verify that the name on the documents was the same as the name of the patient being discharged at the time.
