Veteran A was given wristband for Veteran B. The wristband had full name, full SSN and date of birth. Veteran A relayed the information to his wife who contacted patient advocate and Director. The wristband was removed and the correct one placed at the time of discovery.
Outcome:
03/05/14: The Incident Resolution Team has determined that Veteran B will be sent a letter offering credit protection services.
