Issue:
Veteran A was given Veteran B's echocardiogram report by the Release of Information (ROI) office. An hour later, Veteran A returned the echocardiogram to VA custody. Veteran A stated he saw "female" at the top of the page and did not look any further at the echocardiogram.
Outcome:
04/01/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
