Issue:
Today, 12/19/14, The Outpatient Clinic provided the Privacy Officer (PO) with Veteran As report that he received his medical records along with Veteran Bs medical records. Veteran A requested and received his medical records from the facility by mail. The recovered documents consisted of 290 pages of Veteran Bs medical records. The mailer shows they were addressed to Veteran A. Veteran Bs medical records contained radiology reports, problem list, labs, and medications with his full name, DOB and address, no SSN. Additional notification and investigation to ensue.
Outcome:
12/22/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
