Issue:

Employee's OPF file requested from Augusta VAMC by Charleston VAMC in March. UPS shows mailroom employee signing for item on March 8th. Human Resources never received the package and employee who signed cannot remember what transpired. Three months later the package was sent to Veterans Canteen Service at Charleston VA. Item had been opened and then taped back. So far cannot determine where package has been since March, 2012. Update: 06/19/12:Employee A will be sent a letter offering credit protection services.

Outcome:

Mailroom staff has been given a standard operating procedure (SOP) regarding UPS envelopes coming to the VAMC. The contact person at the UPS center has been advised to destroy all remaining labels listing Veterans Canteen Service as the delivery point for all incoming mail to the VAMC not listing a department or section within the facility. Staff has been advised to refer all mail to supervisor if unable to determine proper destination.Staff has been instructed to contact Privacy Officer if complaint is made for any missing package, letter, etc. to or from mailroom at the time they are made aware of the problem. Privacy Officer will investigate the complaint.
