Issue:
Based on staff interview the facility failed to ensure that Patient 1's clinical information was protected from access by individuals not involved in the patient's care.Findings:On 3/30/12 the hospital reported to the California Department of Public Health that Patient 1's emergency department (ED) records were sent to the wrong place.During an interview on 4/23/12 at 11:15 Administrative Staff A stated that on 3/27/12 Doctor X requested that an ED report for Patient 1 be sent to a local skilled nursing facility. Later that day the skilled nursing facility called the Health Information Department to report that Patient 1 was not a resident at their facility. Administrative Staff A stated that the information should have been sent to another skilled nursing facility with a similar name.The ED report included the patient's name, medical record number, account number, discharge date, medical history, current physical exam information, course of treatment and diagnosis. The patient was notified of the breach of information by a letter on 3/30/12.
