Veteran A called and stated he received a CD with Veteran B's information. He stated the CD contained a name and x-rays that did not belong to Veteran A. Veteran A did have a Release of Information (ROI) request in the system and is worried that another person has received his information. The release of Veteran A's information has not been confirmed (from the request). At this time Veteran A refuses to return the CD.

Outcome:

03/05/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
