Issue:

On September 1, 2011, a funeral home sent a fax to the Marion Release of Information (ROI) clerk for a copy of a DD-214. The ROI clerk faxed a DD-214 and a Honorable Discharge paper to the funeral home for their use.Today the ROI supervisor and the Privacy Officer talked with the funeral home. The information faxed to the funeral home was on the wrong Veteran. The funeral home (not knowing it was incorrect information) used the information for the flag for the family and for the burial expense. Update: 09/24/12:The funeral home was faxed a Veteran's full name and date of birth in error. The Veteran will be offered credit protection services.

Outcome:

The supervisor will review all FOIA-PA cases for the next 90 days prior to the ROI clerk releasing anything and review 10% of all releases per month for the next fiscal year. The supervisor will meet with the ROI clerk at least monthly to re-enforce and continuously review FOIA-PA cases and other releases. The supervisor will document the ROI clerk's training, reviews and findings for the next fiscal year. In the event of a Privacy violation, 100% of reviews will resume.
