Issue:
A VA employee requested a Sensitive Patient Access Report (SPAR). He then requested justification of access by two employees within his service. Update: 04/04/12:The employee will be sent a letter offering credit protection services due to the inappropriate access and viewing of his record, which included exposure of his full SSN and full name.
Outcome:
Service Chief has conducted a fact finding. Appropriate administrative action has been delivered to the two employees. No justification of access to the employee's medical record. Per the SPAR and communication with the Service Chief, he has stated that these two employees would have had access to any/all consults and SC conditions, as well as the fellow employee's demographics, SSN, etc. In addition to the disciplinary action taken by management, it has been recommended that the two employees retake the Privacy/Information Security Training, as well as reviewing and re-signing the National Rules of Behavior. Will offer credit monitoring to the employee. Letter offering credit monitoring mailed to employee 4/6/12.
