Veteran A presented to the office today with an appointment listing for Veteran B. Veteran A alleges that on 12/17/11, upon his discharge from 4South, he requested a copy of his future appointments. He claims that the employee (no name) pulled the appointment listing off the printer and handed it to him.When he looked at it (after he was discharged) he realized it was for Veteran B who was also an inpatient on the floor. Veteran A presented to report the incident. The appointment listing contained Veteran B's name, full SSN and diagnosis. Update: 01/02/12:Veteran B will receive a letter offering credit protection services.
