Issue:
A VA Patient came to the VA Release of Information Office to request medical information be provided to his insurance company and place of work. The place of work was to receive a notice from the patient's care provider concerning the ability to perform work related duties. The information that was to go to the insurance provider contained more detailed diagnostic information.The form that was to go to the insurance provider was faxed to the patient's place of work instead of the insurance provider. The employee came to the VA when the incident was discovered to state that the mistake was causing problems at work. Update: 05/07/12:The patient will be sent a HIPAA notification letter.

Employee responsible for faxing document to be provided with training and education.
