The consultation request included the patients full name, full SSN, DOB, gender, and type of requested procedure.. Update: 11/06/12:Patient B will be sent a letter offering credit protection services due to full name and full SSN being exposed.
Outcome:
Educated the staff on proper safeguard and to ensure to double check what is being mailed to a Veteran.
