Issue:

Veteran A reported to a Nursing Service Administrative Support Assistant that they had been receiving Veteran B's appointment letters and billing information via mail. The letters included Veteran B's date of birth. He also stated that he had been contacted on his personal cell telephone in reference to the Veterans appointments. He also stated that he had informed several staff members that he was receiving this information in error and had returned the mailings to the facility. Update: 05/31/11:Veteran B will be sent a letter offering credit protection services.

Outcome:

Credit Monitoring Letter provided to the veteran, and employees will be counseled to reduce errors in mailings.
