Issue:

Veteran A had a procedure on 04/23/12. When the nurse gave him his discharge information she accidently gave him a piece of paper meant for Veteran B. It contained Veteran B's full name, full ssn and date of birth. It was part of the medication reconciliation. Veteran A destroyed the document when he realized he had someone else's information. Update: 04/25/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

Nurse Manager has sent this up to the Nurse Executive for the final decision on what action to taken. Will be done as deemed appropriate fact finding is complete.
