A Clerk at a Community Based Outpatient Clinic (CBOC) allowed a Veteran's wife to sign VA Form 10-5345a to obtain a copy of her husband's medical records. The wife had no legal authority to sign for the Veteran. Update: 03/12/13:Veteran A will be sent a notification letter.

EMPLOYEE WAS EDUCATED IN THE IMPORTANCE OF OBTAINING CORRECT AUTHORIZATION BEFORE RELEASING PHI. QUESTIONS SHOULD BE FORWARDED TO THE PRIVACY OFFICER OR ALTERNATE.
