A single ID label was left on an EKG machine in the hallway of our nursing home. The hallway is a high traffic area but it is unknown how many people actually saw the label. The label contain full name, full SSN, and DOB. Update: 02/03/12:Veteran A will be sent a letter offering credit protection services.
Outcome:
The label was retrieved and additional training has been provided to the employee.
