Issue:

The VA Police found physical documents in an unsecured drawer. Update: 05/17/11:The documents were located unsecured in the lobby of the medical center, at the information desk. They were unsecured for several days over the weekend. The documents included 56 Veterans' full SSNs. All 56 Veterans will be offered credit protection services.NOTE: There were a total of 15 Mis-Handling incidents this reporting period. Because of repetition, the other 14 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Privacy Officer recommends disciplinary action to employee responsible for not safeguarding PHI.
