Issue:
A Senior RN Specialist-Research Coordinator associated with research study 610-11 reported Veteran A received Veteran Bs medication. The research staff continues to try to contact Veteran A. Veteran B was not affected by this error. Privacy Office will further investigate.
Outcome:
12/24/13: Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
