Issue:

Based on observation of the Floor Nursing Department and staff interview, the hospital failed to ensure unauthorized individuals could not gain access to patient records, which had the potential to result in unauthorized access to patient records. Findings: During a concurrent interview and observation of the fifth floor Nursing Department with Risk Management Registered Nurse (RMRN) on 10/16/12 beginning at 11 AM, it was observed that a physician was documenting on an electronic device at a desk in the hallway. The twenty-inch computer screen had no privacy screen attached to it so patient information was fully visible to any person walking by. Further observation showed multiple screens with patient information fully visible to any one that stood behind the nurses documenting protected patient information on the computers. The RMRN stated the facility had privacy screens at one point, but they may have been removed with the recent computer changes.
