Issue:

An appointment was scheduled for the wrong Veteran which resulted in the patient receiving the other Veteran's information. the information disclosed to the Veteran included full name, last four digits of the SSN and upcoming appointments. The other Veteran hasnt been seen here since 1999 and lives in Michigan. They have the same initial of the last name (names also similar) and same last four.

Outcome:

12/12/14: The Incident Resolution Service Team has determined that the Veteran whose information was disclosed will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
