Issue:

Based on record review, the hospital failed to safe guard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the facility Unusual Occurrence Report dated 5/15/13. the report indicated that on 4/26/13, Patient A presented to the facility. While registering the patient Emergency Room Registration Staff A (RS A) did not up-date Patient A's billing information. This resulted in the bill being sent to the previous guarantor listed on his account which was Patient 1's Mother. The billing information contained the following: Patient 1's name, insurance status, laboratory procedures, account number and total charge. RS A received one on one training to ensure the error does not reoccur.
