Issue:

A timekeeper's list of employees' full names and full SSNs was found on a ward printer. The printer was located in an unsecured area. The list was discovered on the morning of 06/25/12 but was printed out either Saturday 06/23/12 or Sunday 06/24/12. Update: 07/06/12:Fact finding has revealed that the employee who printed the list to the wrong printer was aware that she printed it, but didn't know where it went, and didn't tell anyone that it was possibly missing. It was found on a busy hospital ward counter 2 days after it was printed. It was not on the printer. Someone had picked it up and placed it on a nearby counter. The list contained 19 employees full SSNs and other information. The PO is unable to determine who saw the list in the two days after it was printed. All 19 employees will be offered credit protection services.

Outcome:

We have implemented additional training for admin staff to ensure this issue does not occur in the future.
