Issue:

During the Environment of Care Rounds on Mountain View Living Center (Nursing Home Unit), the Information Security Officer (ISO) entered the old South Nurses station. This station is used by staff on an infrequent basis. The station has counters on two sides so it is open access. There is also a locked door but the ISO was able to reach around from the _outside to open the door. There were papers lying around a computer and the ISO went through them. Most of them were policies, procedures, medical information and the like. However three of the documents contained patient information.Document #3Inpatient Appointment List printed 02/12/2013This list includes the full name and full Social Security Number for seven patients as well as clinic they are to visit and the time of the appointment. Update: 02/13/13:Seven (7) Veterans will be sent letters offering credit protection services.

Outcome:

The nursing home staff has implemented a process that in their regular unit rounding that the door is shut, locked, and that it is a document free zone. This will occur x8 times per 24 hours.
