Issue:

On the afternoon of Wednesday, 12/10/14, pharmacy received a call from Veteran A stating that she had not received her UPS package that was previously set up to be delivered on Tuesday 12/09/14. Upon researching the UPS log, pharmacy discovered that Veteran B had been sent 2 UPS packages (Veteran B's and Veteran A's). Veteran B was contacted by phone Wednesday afternoon 12/10/14 and confirmed that he (Veteran B) had received two UPS packages on Tuesday 12/09/14. Veteran B stated he had not looked at the names on the packages or medication when delivered. Veteran B read the medication labels while on the phone with pharmacy and confirmed that one contained Veteran's A medication and paperwork.

Outcome:

12/11/14: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
