Issue:
A Veteran discussed with the Privacy Officer (PO) a concern over his 7332 information being released to an outside provider. Upon review of the request it was noted that Release of Information (ROI) released in full (including 7332) to the outside provider. The PO attempted to view the authorization in CPRS but it was not scanned. The PO contacted the ROI supervisor to locate authorization. After search by ROI staff, authorization cannot be located. It cannot be confirmed if the Veteran signed for 7332 or for any part of his record to be released to provider.
Outcome:
02/04/14: Veteran A will be sent a notification letter.
