Issue:
Based on interview and record review, the facility failed to ensure that Patient 1's protected health information (PHI) was kept confidential from anyone not authorized access.Findings:An interview with the manager of the emergency department (MED) was conducted on 10/18/12 at 3:45 P.M. The MED stated that Patient 1's discharge instruction sheet was inadvertently given to Patient 2's (minor patient) mother during discharge. According to the MED, the staff was multi-tasking and was processing multiple patients when the error was made. Patient 1's discharge instruction sheet contained the patient's name, date of birth, medical record number, chief complaint, lab and diagnostic tests performed without the test results.
