Issue:

On, 7/23/2013, Pharmacy Supervisor reports to Privacy Officer (PO) that Veteran B was discharged yesterday with her home medications (brought in from home) and also received Veteran As home medications. Veteran B is a Mental Health Intensive Case Management (MICM) patient; social worker returned medications back to the facility pharmacy. Veteran A is still an in-patient. The Pharmacy supervisor initiated investigation to determine that Veteran As home medications were not processed from the unit to Pharmacy in the usual manner; not controlled medications. Both patients were on the same unit at the same time. Veteran B did see Veteran As full name, full SSN, DOB, medication, provider, and medication instructions. Refill of Veteran As home medications by Pharmacy is in process. Additional notification and investigation, pending.

Outcome:

07/19/13: Veteran A will be sent a letter offering credit protection services.
