Issue:
Veteran A came to the VA medical center to pick up a prescription at the pharmacy. Veteran A was given Veteran B's medication. The bag label and prescription bottle had Veteran B information but the paperwork was for Veteran A. Veteran A was contacted and returned the medication and bag within a three hour timeframe. Update: 04/22/13:Veteran B will be sent a notification letter.NOTE: There were a total of # Mis-Handling incidents this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Pharmacy staff were reminded on safeguarding personal health information when dispensing medications. AMG
