Issue:
Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality of Patient A, when a contracted physician accessed personal information on Patient A that were not associated with the physicians practice.Findings:On 1/4/12 at 10:00 AM, an interview was conducted with the Compliance Vice President Officer (CVPO). The CVPO stated that on 10/14/11, he was notified that a physician was accessing a patient face sheet that was not associated with his practice. The CVPO stated that the physician was contacted and was re-educated on the importance of confidentiality.Review of the information of Patient A that was accessed by the physician included the following Protected Health Information (PHI): Patient Name Address Phone number Insurance Type Sex Date of Birth AllergiesThe CVPO further stated that the physician stated that his billing company accessed the information without his knowledge. The CVPO stated that Patient A was called to verify if this physician was her Doctor. The CVPO stated that Patient A stated, "I don ' t know that Doctor. "
