Issue:

An ongoing VA OIG criminal investigation being coordinated with local law enforcement produced one page of a VISTA Inpatient list printed on 04/20/11 at 7:32 AM. It was found in a box of the suspects belongings turned over to law enforcement. The facility was told only this one page of a multi-page list was found. The IG Investigator has provided the Privacy Officer with a copy of the list which contains the following information for 29 Veterans:Full name.Last 4 digits of the Veterans SSN.Date of their admit to the facility as an inpatient.Room location at the time the list was printed.The number of days they had been inpatient at the time the list was printed..The title of the specialty group treating the Veteran. The facility is handling this information very carefully as the initial investigation is still progressing. The investigation is directly associated with NSOC ticket SPE 62495. Update: 07/15/11:Notification letters will be sent to 29 Veterans

Outcome:

Notification letters for this incident were sent in the outgoing mail Oct 6, 2011. 5 to Next of Kin, 24 directly to Veterans. The VA Office from which the document was taken is now a controlled space where only authorized personel are permitted to enter.
