Issue:
On 11-22-11 an ER provider contacted the Chief Pharmacist to report that while completing the medication reconciliation, the patient stated that they had received another patients medication in the mail. CMOP tracking shows that the medication was delivered to the address on file on 11-29-10. A Patient Audit List Report revealed that on 11-22-10, an MAS clerk edited the intended veterans address to reflect the unintended recipients address. This change caused the erroneous delivery and mishandling of this medication. THIS WAS NOT A CMOP ERROR. PO will investigate. Update: 02/24/11:Patient will receive a letter offering credit protection services since their full SSN was exposed.
Outcome:
The PO educated the staff member about being more cognizant when making changes/correction in the patient files.Warn that these are avoidable privacy violations and will not be allowed.
