Issue:

Based on interview and document review, the hospital failed to ensure that 1 of 1 sampled patient's (Patient 1) protected information was kept confidential from anyone not authorized access. Protected information should not be released to anyone without proper authorization.Findings:An interview with the Director of Risk Management (DRM) was conducted on 5/1/13 at 2:00 P.M. The DRM stated that Patient 1 and Patient 2 were both discharged on the same day. The DRM stated that an ambulatory care staff mistakenly gave another patient (Patient 2) Patient 1's colonoscopy photograph. According to the DRM, the colonoscopy photograph indicated Patient 1's name, date of birth, and medical record number. The DRM stated that Patient 1's colonoscopy photograph was returned and was given to the patient. The DRM also stated that Patient 1 was informed regarding the inadvertent disclosure.
