Issue:

Based on interview and record review, the facility failed to prevent unauthorized access to a patient's (Patient A) PHI (Protected Health Information) when the patient's laboratory results were given to another patient.Findings:On May 17, 2013, at 1:40 p.m., a review of the lab requisition for Patient A dated May 3, 2013, revealed, Patient A's name, gender, date of birth, address, medical insurance information, and account number. On May 6, 2013, the facility was informed the lab requisition was given to another patient who was able to view Patient A's Protected Health Information (PHI).In an interview with the executive secretary, she stated, "It was a physician that didn't follow the policy."
