Issue:

An envelope was returned to the facility that had a label with a patient's name, full SSN and DOB affixed to it in place of an address label. The envelope had been franked by the local mail sorting facility but was apparently kicked out at that point since it had no address on it. It appears that the sorting facility also opened the envelope (I'm thinking to try to see who to send it to). Enclosed was an application for benefits which would have had some financial information on it. Update: 09/06/12:One Veteran will be sent a letter offering credit protection services due to full name and full SSN being disclosed.

Outcome:

Staff reminded to use caution when handling documents with PII.
