Issue:
Veteran A contacted facility stating that he received a C&P packet from Veteran Evaluation Service (a supposed contractor for C&P exams) for Veteran B. Privacy Officer will investigate. Veteran A stated that they did talk to Inspector General because that was who he was directed to who told him to contact facility. Patient will take packet to Texarkana CBOC on Friday as he has to work all week. He is concerned about his information being sent to wrong Veteran and wants to have C&P exam at the VA not contracted out.
Outcome:
09/16/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
